Recommended Decision that the Berkeley County Public Service Sewer District is granted Commission authorization to physically terminate sewer service to Customer of record; that at least seven days prior to the date it intends to terminate sewer service, the Berkeley County Public Service Sewer District mail a completed notice attached as Attachment A to Customer of record by first class mail, and mail a completed notice attached as Attachment B to the Berkeley County Health Department and the West Virginia Bureau for Public Health by first class mail; that the Berkeley County Public Service Sewer District make at least two attempts to notify Customer of record by telephone call and, if the sewer utility fails to reach Customer of record by telephone, visit the residence at least 24 hours prior to physically terminating the sewer service; etc. |
